Epidermal vs Dermal Pigmentation: Why Some Pigmentation Is Easier to Treat

Not all pigmentation behaves in the same way.

Some dark marks fade relatively quickly with the right combination of skincare and professional treatment. Others improve much more slowly, even when every recommendation is followed carefully. This often comes down to one important factor: where the pigment is located within the skin.

Understanding the difference between epidermal and dermal pigmentation helps explain why some cases respond quickly, while others require a more gradual, long-term approach.

Where does pigmentation sit?

The skin is made up of multiple layers, but when discussing pigmentation, the two most important are the epidermis and the dermis.

The epidermis is the outer layer of the skin. It is where melanocytes produce melanin and where normal skin cell turnover takes place.

Beneath this lies the dermis, which contains collagen, elastin, blood vessels, nerves and many of the structures responsible for maintaining healthy skin.

Pigmentation can develop within either of these layers, or sometimes in both at the same time.

Epidermal pigmentation

Epidermal pigmentation is located within the upper layers of the skin.

Because this pigment sits closer to the surface, it is gradually carried upwards as skin cells naturally renew themselves. This means it is generally more responsive to treatment.

Epidermal pigmentation often appears:

  • Medium to dark brown.

  • Well defined.

  • More uniform in colour.

  • Easier to distinguish from the surrounding skin.

Conditions such as superficial sun damage and some forms of post-inflammatory hyperpigmentation often fall into this category.

Professional treatments and carefully selected home care can usually produce noticeable improvement because the pigment remains within the part of the skin that naturally renews itself.

Dermal pigmentation

Dermal pigmentation is different.

Instead of remaining within the epidermis, some melanin drops into the deeper dermis, where it is taken up by specialised immune cells called melanophages.

Melanophages are not harmful. They are part of the body’s normal defence system.

Their role is to clean up material that does not belong in the dermis. When melanin enters this deeper layer, melanophages engulf it in an attempt to protect the skin.

The challenge is that these cells are very good at holding onto pigment.

As a result, dermal pigmentation often appears:

  • Grey-brown.

  • Blue-grey.

  • Less sharply defined.

  • Softer around the edges.

  • More diffuse.

Because this pigment sits below the normal process of skin renewal, it cannot simply be exfoliated away.

A useful way to picture it

Imagine spilling ink onto a sheet of paper.

If the ink remains on the surface, it is relatively easy to remove.

If the ink soaks deep into the paper, removing it becomes much more difficult without damaging the page itself.

Pigmentation behaves in much the same way.

Surface pigment generally responds more quickly because it is closer to normal skin turnover.

Deeper pigment requires patience because the skin has to remodel gradually while encouraging melanophages to slowly release the pigment they are holding.

Mixed pigmentation

Most people do not fit neatly into one category.

Many cases of facial pigmentation, particularly melasma, contain both epidermal and dermal pigment.

This is known as mixed pigmentation.

Some areas may appear darker with clear borders, suggesting more superficial pigment. Other areas may look softer, greyer or less defined, indicating a deeper component.

This explains why one part of the face often improves faster than another.

It also explains why pigmentation may lighten significantly without disappearing completely after only one or two treatments.

Why deeper isn’t always darker

One common misconception is that darker pigmentation must automatically be deeper.

In reality, this is not necessarily true.

Very dark pigmentation can still sit within the epidermis and respond well to treatment.

Conversely, relatively subtle grey or blue-grey pigmentation may represent deeper dermal involvement and therefore require much more time to improve.

The colour, border definition and overall appearance often provide more useful information than darkness alone.

Why aggressive treatment isn’t the answer

When pigmentation is slow to improve, it can be tempting to assume that stronger treatments will produce faster results.

Unfortunately, this is often the opposite of what happens.

Pigmentation is closely linked to inflammation.

Aggressive peels, excessive exfoliation or overly stimulating treatments can increase inflammation within the skin. In turn, that inflammation may encourage melanocytes to produce even more pigment.

This is one reason why experienced practitioners often take a more conservative approach, particularly when treating melasma or darker skin tones.

Improving skin health gradually usually produces better long-term results than trying to remove pigment as quickly as possible.

The skin has to let go of deeper pigment

One of the most important concepts to understand is that dermal pigmentation cannot simply be forced out of the skin.

Instead, treatment focuses on creating an environment where the skin functions more normally.

This includes:

  • Reducing chronic inflammation.

  • Supporting the skin barrier.

  • Improving healthy skin renewal.

  • Protecting against ultraviolet exposure.

  • Reducing unnecessary irritation.

  • Using appropriate pigment-regulating ingredients.

As the skin becomes healthier, it becomes better able to remodel itself over time.

That gradual process is often what leads to improvement in deeper pigmentation.

Realistic expectations lead to better results

Understanding the depth of pigmentation allows treatment plans to be based on science rather than unrealistic expectations.

Superficial pigmentation may improve relatively quickly.

Mixed pigmentation often requires several treatments.

Dermal pigmentation almost always requires patience.

None of this means treatment is unsuccessful. It simply reflects how the skin heals.

Looking beyond the pigment
